Comprehensive Outpatient Treatment for Adult Men and Women

The Transformation House treatment network offers outpatient treatment for men and women 18+ through our Recovery Hub. Outpatient programming, similar to residential treatment, is structured and comprehensive, offering individual and group counseling, as well as dual diagnostic and psychiatric care. Additionally, outpatient treatment affords individuals the flexibility to continue to work, begin living at home, or to maintain daily responsibilities while receiving the care they need in order to prevent a relapse.

Providing Different Levels of Individualized Care for Substance Use and Mental Health Disorders

At Transformation House we help individuals transition back to meaningful lives by creating an environment where change is possible. Clients learn coping skills to help them become active members of their community and identify substance abuse triggers to avoid in order to maintain sobriety.

We have different outpatient co-occurring treatment tracks and programs which allow clients to progress at their own pace while following their own individualized treatment plan.

Our treatment centers and clinics provide a seamless continuum of care, guiding individuals through a comprehensive range of services. From residential high intensity and low intensity programs to outpatient partial hospitalization, intensive outpatient, and non-intensive outpatient care, we are equipped to support your recovery journey at every step.

Maintaining Sobriety after Residential Treatment

The longer someone remains in treatment, the greater the chances of sustained sobriety. Although relapse may be part of the recovery process, the primary purpose of extended care is to offer individuals the tools, resources and support system to remain abstinent from drugs and alcohol use disorders, prevent relapses, and improve their quality of life. After many years of substance abuse or drinking, it is unlikely that a complete behavioral change will occur in 30, or even 90 days of residential treatment.

In fact, the initial period after leaving residential treatment presents the highest risk for relapse. Transitioning from the confines and restrictions of an addiction treatment center and returning to a life of minimal structure and support puts people at risk of relapse during this period. Outpatient programs help a person deal with stressors and triggers that come with responsibility and freedom.

Outpatient High Intensity and Intensive Outpatient (IOP) Programs

Our Outpatient High Intensity program offers the highest level of care in a non-residential setting, with 20+ hours of treatment services per week. For those who require a slightly less intensive level of care, our Intensive Outpatient treatment provides 9-19 hours of programming per week.

These programs offer a range of therapeutic interventions, including group therapy, individual counseling, addiction education, and life skills training. Our clients benefit from a holistic approach that addresses various aspects of their well-being, such as stress management, career development, and budgeting. Additionally, our outpatient programs ensure access to Certified Peer Recovery Specialist (CPRS) services and psychiatric care on a weekly basis, catering to the individual needs of our clients.

Breaking Barriers to Recovery: Transformation House's Integrated Care Model and Sober Living Residences

Outpatient with Lodging

With the goal in mind to remove the housing barrier faced by many in early recovery, Transformation House model of care includes long-term recovery residences. Clients enrolled in a Transformation House’s High Intensity Outpatient track, receive a credit of up to $650 per month towards program fees in one of our sober living facilities or in an independently managed sober house part of TH recovery residences network. Our sober houses are conveniently located in safe neighborhoods of the North Metro Area, close to our outpatient counseling clinics.

Our outpatient treatment program is designed for individuals who have maintained a level of stability and achieved a period of uninterrupted sobriety. It also may be the right choice if you’ve recently completed a stay at an inpatient treatment center. The frequency and intensity at this level of treatment provide ongoing support and connection as clients complete their transition from a higher level of care. By the end of an outpatient program, clients have successfully built an external support network, are actively practicing sobriety maintenance skills, and have implemented their community-based recovery plans.

Non-intensive outpatient treatment generally consists of 2-8 hours of programming per week comprised of a combination of group and individual work. Relapse prevention, relationship maintenance, and ongoing spiritual development are key areas of focus during this phase.
